Hi friends and family,
I'm sorry to those I have been out of touch with in these past few weeks. For those who do not already know, we had a very traumatic birth experience here in the DR as the doctors made some poor medical decisions and we have been trying to recover from this experience. Obviously this post will not be a part of Kenya's baby book we are making, but rather we wanted to inform people about what is going on and ask for your help.
This past Saturday night, Kenya began having seizures during the night. We suspected she was having seizures before this but we were not sure. We went into the ER Saturday night in Santiago but waited for 10 hours without seeing a doctor and they wouldn't take us seriously, saying that she was just making "facial gestures" and refusing to check her. So we left Santiago and went to Santo Domingo, the capital, where Dave's brother Tim and his family live and they have been helping us find people to see Kenya. On Monday she saw a ped. neurologist, general neur., and pediatrician, who all confirmed that she was having seizures and said they believe it was from the trauma during birth. We ordered some tests to get done the next day and the neur. asked us to stay up during the night tracking and timing her seizures. She had 2 seizures in a row Monday night that lasted more than 20 minutes each and the neur. said this was a medical emergency and to get her into the ER in Santo Domingo. They put her in the NICU for the next 34 hours which made those the worst 34 hours of our lives (if you can imagine what it's like to leave your baby in a NICU already, try to imagine the hell of leaving your baby in an NICU in a developing country). They put her on 5 medications which left her as a completely different baby, all drugged up, extremely sedated and unhappy. This has been killing us to see her like this. Kenya went through a bunch of tests including cranial ultrasound, cranial MRI, an EEG, blood tests, hypoglicemia, etc. on Tuesday and Wednesday. We also had a test sent to the states to test for metabolic and congenital causes of epilepsy and are waiting for those results.
The tests from the MRI and EEG came back to confirm she has generalized secondary neonatal seizures. They believe this is related to the trauma that occurred during birth. Finally they released her from the NICU and sent her home on Keppra anti-seizure medication and told us she would need to take this med for a minimum of the next 5 years. They make her extremely sedated and unhappy, and she has no muscle tone. We are checking all the time just to make sure she is still breathing. It breaks our heart because we feel like she is like an entirely different baby. She also continued to have seizures even with this medication. We called the doctor last night and she doubled her dose of this med last night. So even though the med. is not recommended for infants under 1 month, and a baby of her weight should be taking approx. 40 milligrams a day, Kenya is taking 200 milligrams a day - 5 times the recommended dose for her weight. We were told not to worry about the side effects right now of what this is doing to her, but just try to get the seizures under control because the seizures are what is most dangerous to her right now. But the worst part is that even on all this medication, she is continuing to have seizures and had them throughout last night and this morning. We don't know what to do.
Dave and I have been praying and praying and praying for Baby Kenya. We don't know what else to do at this point to help her. We have never been so scared or discouraged in our lives. We keep praying that at some point we will wake up and realize this was all a bad dream.
We are trying to declare medical emergency so we can speed up her passport and somehow get her to the states. We don't have health insurance for her in the states so we are trying to find hospitals that offer free children's healthcare. We have been recommended Shriner's Children's Hospital in Memphis, Tennessee but would love to hear more ideas and suggestions from anyone who has them. We are afraid applying to get on Medicaid would take too long.
We are seeking advice and suggestions from anyone who knows anything about epilepsy in children and what we can do. Dave and I are staying up around the clock to monitor, time, and record her seizures but we have a lot of questions about whether we are doing this correctly. If you know about seizures and are able to help us with this, please let us know. If you have contacts with any ped. neurologists in the states we can talk to, we would be thankful to have a contact like this.
We are not seeking your sympathy with this situation but rather just ask for tons of prayer for Kenya and for our family. Dave and I are really run down and struggling. I had severe damage from the implications of the birth and it has gotten a lot worse and infected as I have been pushing myself this last week to help Kenya. I will probably need to get surgery to repair this sometime in the future. Dave is sick with what may be some kind of parasite and is really run down. Thankfully we are staying with his brother Tim and his family who live in the capital and they are taking good care of all of us. For the time being, we are going to stay here in the capital where Kenya has access to better medical care and we do not know when we will return to Jarabacoa at this time.
Please understand that we do not have time to respond to personal emails and phone calls at this time, unless they are medically related. We need to put all our time right now into helping Kenya get her seizures under control and getting her the best healthcare possible.
Also, if you feel led to make a financial donation to help with Kenya's medical expenses, please let us know. We have already maxed out all of her health insurance that is covered under my insurance plan, and need to get her on a new plan for the rest of her time here. In the meantime, medical expenses are coming out of pocket and neither of us are able to work at this time. If you feel led to help us out with these expenses, please let us know.

I am so very sorry to hear about everything that is happening with your family and sweet baby Kenya. I wish I could help in other ways but until then, my thoughts and prayers are certainly with you. I am not sure when you will check these comments or if this is the best way to get in touch with you but I spoke with my dad who is a GP in Canada and he does think the does seems very high for a baby that young, and if she is still having seizures on that much, upping the dose isn't the answer at this point. He says a baby that young being so sedated is not a good thing. He says that drug they have given is quite new (about 3 years old) and still not usually used on babies so young. That one of the older, better known seizure drugs may be a better option. He is going to speak to some people and do some research and get back to me if he can help in any other way. My brother is also a doctor in Nova Scotia so I will see if he has any contacts as well. I have a pediatrician who works part time in Centro Medico Cabarete and also a couple mornings here in Cabrera. We really like her and she seems to know her stuff... maybe not to this extent as it may be beyond her expertise but I don't know. It may be worth speaking with her. I also believe the head doctor at Centro Medico Cabarete is one of the top neurologists in the country. I am going to try to get some phone numbers for you then I'll come back here and comment again with those. Thinking of all of you.
ReplyDeleteOkay, I spoke with my dad again and he says upping the dose of Keepra so quickly really is not the best option. For a baby her age, about 28 mg a day, twice a day is all she should have. After that it should only ever be upped every 2 weeks due to the side effects, definitely not after just a day or two, especially on babies so young. If it isn't working, instead of upping the dose they should try a different medication. He suggested phenobarbital.. which is the old standard, predictable and safest stand by. He suggested definitely getting a second opinion from what you have been giving so far as far as medication is concerned. Also asked, is she still eating well? Are there times she is lively or always very sedated?

ReplyDeleteI am a friend of jodi chandlers sister and she told me about your blog. First of all I will be praying for you! I am a nurse on a seizure unit at children's hospital in st Paul Mn. Our Dr.s often use keppra as a first option for new onset seizures. There are different meds that work on different kinds of seizures. Keppra is a med that covers a lot of kinds it is a newer med but there are not a lot of side effects. It shouldn't be causing Kenya to be so sedated...is she on others medicines? I think the drs have the right idea in wanting to get the seizures under better control. Phenobarbital is an old medicine with a lot of icky side effects...I do think it is sometimes used in newborns but our drs try not to use it. Again I am just a nurse not a doctor....it seems like the right tests have been done...seizures are tricky and sometimes hard to get under control...there are many medications and even possible surgery options down the road. I am a mom too and my heart is with you in wanting your baby cared for and back to herself. You are being a great advocate for your child in staying informed, asking questions and of course PRAYER!

ReplyDeletei got this information from the website for the US embassy in santo domingo: "If you have an emergency involving the death, arrest or hospitalization of an American citizen, or a missing American citizen, please call 809-221-2171. When the prompts ask you to enter the extension, enter 0 and tell the operator you have an emergency."
I would also encourage your family stateside to be calling their sate and federal house members to get their staff on this asap too. The house seems to have staff that responds much faster than the senate. Once in the states, it is federal law that a hospital must not deny you medical care. Bills you will deal with later! Also once, in the hospital, the social workers there will help you access what you need. Top Children's Hosp are certainly Shriners, but also Children's in Philadelphia and Pittsburgh.

I don't know of a Shriner's hospital in Memphis, but Le Bonheur (in Memphis)is "home to one of the most advanced epilepsy centers in the country, staffed by pediatric neurologists with special training in neurophysiology. This serves as a regional and national center offering many therapeutic approaches for children with epilepsy, including medication, neurosurgery and special diets."

Hi! I am Tony's wife, former nph volunteer. I am a case manager for medicaid...can o all what your home state is? In Wisconsin, the medicaid process only requires the following thing baby's birth certificate/passport, SSN, and proof of WI residency. Medicaid is a federal program, but each state has ours own eligibility rules. Feel free to find me on Facebook me and I can inquire about other state agencies. Also, I do have medical contacts in the DR, mostly on the East, but if you ate open to contacting them, you are more than welcome to do so. Praying for you guys. ReplyDeleteHi, I am friends with Kristy Taylor and I saw this link on her facebook page. I'm so sorry you are going through this and you will most certainly be in my prayers. This may sound unusual if it isn't anything you are familiar with, but I felt the urge after reading this to tell you about some of the wonders of chiropractic. I have personally been seeing a chiropractor for the last 6 months for a number of issues and I can't tell you enough great things about it and how it has helped me. One thing I have learned in doing so is that traumatic birthing experiences are a major and common issue. It amazes me how God has connected everything in our body to our spine, and sometimes if the spine is out of whack, (like from being pulled out of a body by your head), bad things can happen. I know epilepsy/seizures especially are very common results of poor birthing experiences and maybe seeing a chiropractor could significantly improve Kenya's health, if not completely rid her of seizures, (I have heard if happens a lot). At the least, maybe it could help her feel a little better and not need as much medication. I'd say you don't have anything to lose and it's natural so you don't have to worry about side effects, Also, I'm sure compared to the treatment you've already been seeking it's relatively inexpensive. Hello. My name is Gary. I am a pediatrician in KY and a childhood friend of Glenn Harden. (Rachel is my wife.) I would presume that the seizures are the result of brain injury due to lack of oxygen during the traumatic birth process. However, they may also commonly be due to prenatal infection or an infection that developed during or after delivery. It could also be due to a stroke, possibly suffered during the trauma. Incidentally, did the brain scans show anything? Treatment of seizures is often very difficult and may require more than one medication. Medications are often used that have not been "approved" for kids or at doses that have not been approved for kids. However, I agree with a previous comment in that the baby probably should not be so heavily sedated. I do not know anything about the DR or especially their medical abilities, but I would personally do anything in my power to get my baby evaluated and treated in the US. Any large children's hospital should be qualified and capable of taking care of your bably. As someone above mentioned, the hospital cannot turn you away based on your ability to pay. In addition, I think most children's hospitals have foundations to help cover the costs in scenarios such as yours.

Hi Kristin and Dave, I am Maureen, Jaci's mom. After Jaclyn called this morning, I placed a call to Shriner's hospital in Philadelphia. A social worker is supposed to call me back soon. If you are able to get her into this hospital (which doesn't turn anyone away for financial reasons), you are more than welcome to stay at our house. We will be coming to Jarabacoa in two weeks, but the house is yours regardless. We will continue to keep baby Kenya and your family in our prayers!!!!!

We had a son with health issues when he was born and we had him at the Children's hospital of Philadelphia (CHOP). I highly recommend them, we had nothing but great experiences there!
ReplyDeleteHi, I just saw this on Facebook and wanted to tell you that I will be praying for you and your baby girl. My daughter was born March 1st and also has seizures that we are still trying to control and like someone els said, it's really trial and error, our daughter is currently on 3 meds including keppra and phenobarbital. She has different seizures then your baby though. We are seeing a pediatric epileptologist which I highly recommend over at children's hospital in Chicago, but we also went to devos. I pray everynight that there will be a mass healing of babies with epilepsy because it is the worst thing for a parent to watch your baby go through and our sweet babies should never ever have to go through it.
